By providing this information, you are consenting to the RSPCA sharing your details in order to obtain the animal's veterinary history and records.

The person signing this Transfer of ownership form agrees that:

1. The information given in this form is true and complete to the best of his/her knowledge and belief.
2. He/she has full power and authority to enter into this agreement.
3. Where he/she acts as agent for the owner, should his/her authority be subequently questioned by the RSPCA or any third party, he/she agrees and acknowledges that he/she is personally bound by the terms of this agreement, and will additionally compensate any wronged person and indemnify the RSPCA against all claims, liabilities, losses and costs associated with any unauthorised act.
4. The information provided by you will be retained no longer than necessary and may be shared, where relevant, with the RSPCA national society or other branches of the RSPCA. We will not share information with third parties.
5. Upon signing this form, the owner will have no right to receive the animal back or exercise any other rights in respect of the animal. The owner acknowledges the intent of the RSPCA and the owner, recorded in this agreement, that the RSPCA shall be entitled to rely on the terms of the agreement and its ownership of the animal in taking any all actions affecting the animal.
6. He/she is aged over 16 years of age.

Upon signing this agreement the owner hereby transfers full ownership and title in the animal to the RSPCA and the RSPCA accepts the animal into its care and control. In consideration of the RSPCA agreeing to accept the above animal into its care and discharge its obligations as owner to provide the animal with an appropriate standard of care in accordance with RSPCA policy and procedures, the owner hereby irrevocably and unconditionally transfers full title in the animal to the RSPCA.
